When the patient is transferred to another hospital, family members and friends who are already co-writing can continue to write in the diary.
This does not apply to nurses. Contact the helpdesk at help@post-icu.com to allow nurses from the new hospital to co-write. In the email, mention the request and provide the corresponding personal diary code. You will be notified when it is set up, or we will let you know if it is not possible because the hospital does not use the Post-IC diary.
After the transfer, nurses from the previous hospital can no longer write in the diary.
